• Are you submitting as Artist, Manager, Promotor, or Other (if other, please identify).
• Where is the artist from. If outside the continential US, please specify.
• Artist name
• Official Song Title
• Official Album Title
• Songwriter
• Label
• ISCR#
• Cover
• Website and social media sites

Please note that if required information isn’t provided that your submission may not be considered. Song should be a minimum of 128kbps.
